Our Older Adults Mental Health Service is going through an exciting time. We are currently redesigning our existing community based services which include the Community Mental Health Team, who will support people with moderate to complex mental health needs; and the Memory Assessment Service. With investment into our services we have also been able to develop a new Hospital Liaison Service, and to add Mental Health nursing expertise to the existing Rapid Response and Reablement Service, and will support people in order to reduce admissions to hospital. We are introducing a new Primary Care based Community Mental Health Team who will support older people with low to moderate mental health needs and dementia, whose needs can be met with support from their GP.

Our staff are committed to delivering person-centred care, working collaboratively with older adults who experience mental health issues or dementia and their families and carers, to ensure they receive the ‘right care, at the right time, from the right person’. We have close working relationships with colleagues in other parts of the organisation, and  services within  the voluntary sector who are key to helping us deliver care and support focusing on enabling people with on-going mental health issues or dementia to  ‘live well’.

To ensure we offer the best and most up-to-date mental health and social care, we actively encourage continuing professional development. This includes induction for new arrivals to Jersey, free access to online journals and e-books, and ongoing study up to Masters level for nurses and allied health professionals in partnership with the University of Chester.
